Home Medicines Review

Are you uncertain what each medicine is for? Are your medicines making you feel sick or maybe they are not working as effectively as you and your doctor first thought they would?

You may be eligible for a Home Medicines Review, conducted by our in-house accredited consultant pharmacist!

What are some of the benefits of having a Home Medicines Review?

  • Provides an opportunity to have your medicines assessed to optimise your treatment;
  • You can discuss your medicines with a fully qualified consultant pharmacist, to improve your understanding of your medicines and get the best possible response;
  • The pharmacist will provide a report to your doctor who will discuss the findings with you at a follow up appointment.
How much will a Home Medicines Review cost me?

A Home Medicines Review and the follow up GP consultation are fully funded by Medicare – meaning no out of pocket expense to you!

How often can I have a Home Medicines review?

A Home Medicines Review can be conducted every 2 years or as often as required, if you have significant changes to your medication or health (e.g. recently discharged from hospital).

Please contact the practice on 07 3279 0444 and ask to speak to our pharmacist (if unavailable at the time, our practice nurse will arrange a time for the pharmacist to contact you).

Who can I speak to if I have any questions?

Please contact reception and ask to speak to our pharmacist or one of our practice nurses; alternatively, you could speak to your GP at your next visit.
